A "Sports Physical" is needed for ANY student (grades 6 - 8) school sponsored
Intramural or Interscholastic sports activities.

1 - Link No. 1: is the REQUIRED state Physical Examination Form.  No other examination form is acceptable.

You may print this form or come to the school for a copy of this form.

2 - If a student does not have a medical home (primary care physician), the district shall provide  this examination at the school physician's office, free of charge.  Read the "Official Announcement Letter" on how to receive the required "school physician authorization" form to see the school doctor.

You MUST have the "school physician" form to see the school doctor.  
To receive this form, you MUST come to the school office.

This COMPLETED Physical Examination Form is required for
ANY (grades 6 - 8) after-school athletic activity !
